Post-Vasectomy Semen Analysis
Your vasectomy is not confirmed successful until a semen analysis shows zero sperm. This step is not optional — it is the only way to know your vasectomy worked. University Urology offers in-office testing and partners with Fellow for a convenient at-home option.
Why This Matters
Your Vasectomy Is Not Confirmed Until You Test
Do not stop using contraception until your semen analysis is confirmed negative
Vasectomy is highly effective, but it is not immediate. Sperm can remain in the reproductive tract for weeks to months after the procedure. Unintended pregnancy after vasectomy is most often the result of unprotected intercourse before a negative semen analysis is confirmed. Continue using other forms of birth control until your test shows zero sperm.
The AUA recommends post-vasectomy semen analysis at least 8–12 weeks after the procedure and after a minimum of 20 ejaculations. At University Urology, we recommend testing at 12 weeks. In rare cases, it may take longer to clear sperm — your provider will guide you if a repeat test is needed.
Men who skip their post-vasectomy semen analysis have no way to confirm their procedure was successful. Vasectomy failure — while uncommon — does occur, and it can only be detected through testing. Compliance with post-vasectomy semen analysis is one of the most important things you can do after your procedure.
Your Testing Options
Two Ways to Get Your Semen Analysis
We offer two options: bring your specimen in to our office, or use Fellow's at-home kit for added convenience. Both are clinically valid. Fellow is a third-party service with an additional out-of-pocket cost not covered by insurance or included in your vasectomy fee — but for many patients, the privacy and convenience are worth it. What matters most is that you complete the test.
At-Home Option — Recommended for Convenience

Fellow is a trusted at-home semen analysis platform used by urology practices nationwide. Their CLIA-certified lab processes mail-in samples with the same clinical accuracy as in-person testing. The kit is simple to use, ships directly to your home, and includes everything you need. University Urology recommends Fellow for patients who prefer to avoid an in-office collection.
Receive your kit, produce a sample at 12 weeks, register using the QR code, and drop it in any UPS location with the pre-paid label. Results are available within a few business days of receipt.
Fellow's lab is CLIA-certified and their test has been validated against fresh in-person semen analysis. Results meet AUA guidelines for confirming vasectomy success.
If sperm are detected, Fellow provides a free replacement kit for retesting. Your University Urology provider will review results and contact you directly to discuss next steps.
Fellow automatically sends an email reminder at 12 weeks so you don’t forget to test. Studies show that at-home testing significantly improves post-vasectomy semen analysis compliance compared to in-office testing.
What to Expect
Post-Vasectomy Timeline
Most men complete this process without complications. Here is what to expect from procedure day through confirmed clearance.
Procedure Day
Your vasectomy is performed in the office. If using Fellow, your kit may be provided or ordered at the time of your procedure. Continue all other forms of contraception from this point forward.
Weeks 1–11: Allow Time to Clear
The AUA recommends at least 20 ejaculations before testing. This helps clear residual sperm from the vas deferens. Continue using contraception throughout this period without exception.
Week 12: Submit Your Sample
At 12 weeks, produce and submit your semen sample — either in our office or through your Fellow kit. Register your Fellow kit at the time you produce your sample using the QR code inside the box, even if you activated it earlier.
Results
In-office results are reviewed by your provider. Fellow results are available within a few business days of the lab receiving your sample and are delivered directly to you. Your University Urology provider will contact you if any follow-up is needed.
Confirmed Clearance
When your result shows zero sperm (azoospermia), you may stop using other forms of contraception. If any sperm are detected, do not stop using contraception — your provider will advise on next steps, which may include repeat testing at a later date.
What "zero sperm" means
A result of azoospermia (no sperm detected) on your post-vasectomy semen analysis means your vasectomy was successful. Per AUA guidelines, you may then discontinue other forms of contraception. Some providers also accept rare non-motile sperm (<100,000/mL) as an acceptable endpoint — your University Urology provider will confirm what standard applies to your result.
